This premolar had short clinical crown and extensive caries damage making an indirect full coverage restoration difficult. A hard resin index was made of the pre-op cusp tips and occlusal surface of the tooth and it's neighobors. The proximal embrasures were not indexed to allow seating of a matrix band.Endo was completed with a greater curve band in place to aid isolation of the deep distal margin. Because the index doesn't touch the interproximal areas, it will still fully seat with the band in place. The cusp tips were reduced 2mm to allow adequate thickness of composite. The tooth was incrementally built up. With each increment, a piece of teflon tape was placed over it and the index pressed down to ensure the preop anatomy was not distorted. The matrix is easily seated onto the neighboring teethOnce proximal surfaces were fully built, the band was removed and each cusp was stamped in separately utilizing the teflon/resin matrix. Excess was cut back prior to polymerizingPre-op occlusion and cuspal anatomy reproduced- no occlusal adjustments were needed. A fine finishing diamond was used to remove excess flash on buccal/lingual surfaces. Best of all worlds: lower cost, exact anatomy, no excess tooth removed, and single visit treatment!
