Re treatment of a tough upper 6 with multiple complications !
Re treatment of a tough upper 6 , with :- 1) broken file in the apical third of the MB1 ! 2) several previous trials of bypassing this broken file leading to false path that ended up with apico lateral perforation ( see diagram 1 ) >> this scenario made the case very difficult for me to bypass this apically situated file , so i have to give a trial for retrieval :) - firstly proper coronal flaring ( anticurvature ) till the level of the broken file ( see Diagram 2 ) - now - under the microscope - i can easily insert the US tip towards the inner wall beside the broken file ( some prebending of the tip was also useful ) ( see Diagram 3 ) - after several trials , the file was successful retrieved , and i could gain access to the uncleaned part of the canal where the file was broken - now its time for obturation , a master cone is placed in the MB1 canal , a fine system B tip is placed deep " unactivated " , then activate the sysem B and cut the master cone in a direction AWAY from the perforation inorder to prevent extrusion of hot GP from the perforation ( see Diagram 4 ) - the apical part of the canal apical to the perforation is filled with GP , the perforation above is sealed with MTA using fine carriers ( MAP one ) , and the rest of the canal was backfilled with injectable GP ( see Diagram 5 ) Sorry for the long scenario , hope it may help ! :)
