"Robin Hood" approach for Osseodensification crestal Sinus lift with immediate implant placement using an extracted 38 molar as a grafting material.

A 38 molar with a carious root lesion was extracted and then grinded on a Biokometa Dentin grinder device. The dentin particles mixed with APRF were then used as a material for a crestal Sinus lift using Densah Burs on an upper maxilla edentulous site. A Straumann implant was simultaneously placed on the crestal Sinus lift site and finally the 38 molar site was grafted with APRF Sticky Bone.

Initial Situation: Tooth 38 with mesial root carious lesion to be removed. Note the bone loss between 37 and 38.
Initial Situation: Posterior area quadrant 3.
After tooth 38 extraction and socket degranulation with EK burs.
Tooth 38 extracted. Note the root carious lesion on the mesial aspect.
Tooth 38 after cleaning with diamond bur.
After removing the occlusal filling.
Tooth grinded into particles with Kometabio Dentin Grinder following the usual protocol.

Preparing the grafting material for 38 and 16 sites: - APRF liquid Sticky Bone with Xenograft. - APRF liquid Sticky Tooth Bone with dentin particles. - APRF Membranes.
Grafting material all set.
Panoramic View on CBCT: 3 mm Residual bone height on site 16. An Osseodensification crestal Sinus lift is scheduled with Versah Protocol 3.
Site 16 Initial Situation.
Incision design with minimal Invasive approach.
Mini Flap for drilling area access.
Protocol 3: - 3.0 drill 1000rpm 1mm depth in reverse mode. - 4.0 Drill 1000 rpm till sinus floor in reverse. - last drill for pushing grafting material APRF Sticky Tooth Bone till 10mm depth 100-150 rpm in reverse.
Pushing the graft with Densah drill.
Flap closure with 6.0 polypropylaine sutures.
SLA surface and APRF growth factors.
Straumann BL 4.8 10mm implant placed in correct position.
Post op OPG detail site 16.
Site 38 grafted with APRF liquid Sticky Bone (xenograft). Graft covered with 3 layers of APRF membranes and sutured with 5.0 Glycolon.
